VNOTES (vaginal natural orifice transluminal endoscopic surgery) – инновационный метод эндоскопической хирургии, при котором доступ к органам брюшной полости осуществляется через влагалище. Актуальность изучения робот-ассистированной vNOTES-гистерэктомии обусловлена необходимостью объективной оценки ее преимуществ и возможности внедрения в широкую клиническую практику, а также потенциального расширения показаний.</p><p>Цель исследования – проанализировать эффективность и безопасность робот-ассистированной гистерэктомии с использованием трансвагинального эндоскопического доступа.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены 5 пациенток с ожирением и отсутствием тяжелого пролапса органов малого таза. Ретроспективно оценивались следующие данные: показания к операции, интраоперационные характеристики, объем кровопотери, интенсивность послеоперационной боли, потребность в анальгетиках, частота осложнений.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст пациенток составил 51,6 ± 4,98 [46,5; 56,0] года, средняя продолжительность операции – 106,8 ± 14,25 [95,0; 122,0] мин. Ни в одном случае не потребовалось перехода к лапаротомии или конвенциональной лапароскопии. Объем кровопотери составил 104,0 ± 46,15 [67,5; 150,0] мл. Уровень боли через 6 и 12 ч после операции в среднем равнялся 5,0 ± 0,71 [4,5; 5,5] и 3,2 ± 0,45 [3,0; 3,5] по визуально-аналоговой шкале соответственно. Послеоперационное пребывание в стационаре длилось 5 дней. В данной серии случаев не было выявлено ни одного осложнения как в ходе оперативного вмешательства, так и на протяжении 6-месячного периода послеоперационного наблюдения.</p></sec><sec><title>Заключение</title><p>Заключение. Робот-ассистированная vNOTES-гистерэктомия открывает новые горизонты в области малоинвазивной гинекологии и заслуживает дальнейшего внедрения в клиническую практику.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Vaginal natural orifice transluminal endoscopic surgery (VNOTES) is an innovative technique of endoscopic surgery in which abdominal organs are accessed through the vagina. The relevance of studying robot-assisted vNOTES hysterectomy is driven by the need for an objective assessment of its advantages and the potential for its introduction into widespread clinical practice, as well as identification of potential areas for more indications.</p></sec><sec><title>Aim</title><p>Aim. To analyze efficacy and safety of robot-assisted hysterectomy using transvaginal endoscopic access.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 5 patients with obesity and absence of severe prolapse of the pelvic organs. The following data were retrospectively assessed: indications for surgery, intraoperative characteristics, blood loss volume, postoperative pain intensity, requirement for analgesics, complication rate.</p></sec><sec><title>Results</title><p>Results. Mean age of the patients was 51.6 ± 4.98 [46.5; 56.0] years. Mean operative time was 106.8 ± 14.25 [95.0; 122.0] minutes. No conversions to laparotomy or conventional laparoscopy were required. Blood loss volume amounted to 104.0 ± 46.15 [67.5; 150.0] mL. The VAS score assessed at 6 and 12 hours postoperatively was 5.0 ± 0.71 [4.5; 5.5] and 3.2 ± 0.45 [3.0; 3.5], respectively. The postoperative hospital stay was 5 days. In this case series, no complications were identified either during the surgical procedure or during the 6-month postoperative follow-up period.</p></sec><sec><title>Conclusion</title><p>Conclusion.
